Doc groups issue list of overused tests, therapies

(AP) ? Don't be afraid to question your doctor and ask, "Do I really need that?"

That's the advice from leading medical groups who came up dozens of tests and treatments that physicians too often prescribe when they shouldn't.

No worrisome stroke signs? Then don't screen a healthy person for a clogged neck artery, the family physicians say. It could lead to risky surgery for a blockage too small to matter.

Don't routinely try heartburn medicine for infants with reflux, the pediatric hospitalists say. It hasn't been proven to work in babies, and could cause side effects.

Don't try feeding tubes in people with advanced dementia, say the hospice providers. Helping them eat is a better option.

These are examples of potentially needless care that not only can waste money and time, but sometimes can harm, says the warning being issued Thursday from medical specialty groups that represent more than 350,000 doctors.

Too many people "think that more is better, that more treatment, more testing somehow results in better health care," said Dr. Glen Stream, former president of the American Academy of Family Physicians, which contributed to the list. "That really is not true."

The recommendations are part of a coalition called Choosing Wisely, formed by the American Board of Internal Medicine Foundation. Participating medical societies were asked to identify five tests or treatments that are commonly overused in their specialty. The list is aimed at doctors and includes references to published studies. Consumers Reports and other consumer groups are publicizing the information in more patient-friendly terms.

Last year, the coalition listed 45 overused tests and treatments. It included some of the best known examples, such as too much imaging for back pain and repeating colonoscopies too frequently.

This year's list adds 90 more overused kinds of care. Some are the result of doctors' habits, hard to overcome despite new evidence, Stream said. Others come about because patients demand care they think they need.

Some other examples:

?Don't use opioid painkillers for migraines except as a last resort, say the neurologists. There are better, more migraine-specific drugs available without the addictive risk of narcotics. Plus, frequent use of opioids actually can worsen migraines, a concept known as rebound headache.

?Just because a pregnant woman misses her due date, don't race to induce labor if mom and baby are doing fine, say the obstetricians. Inducing before the cervix is ready often fails, leading to an unneeded C-section. "Just being due by the calendar doesn't mean your body says you're due," Stream notes.

?Don't automatically give a child a CT scan after a minor head injury, say the pediatricians. About half of children who go to the ER with head injuries get this radiation-heavy scan, and clinical observation first could help some who don't really need a CT avoid it.

?And don't leave an implanted heart-zapping defibrillator turned on when a patient is near death, say the hospice providers. This technology clearly saves lives by guarding against an irregular heartbeat. But if someone is dying of something else, or is in the terminal stages of heart disease, it can issue repeated painful shocks, to no avail. Yet fewer than 10 percent of hospices have formal policies on when to switch off the implants.

Kansas City restaurant fire rubble searched for missing woman

KANSAS CITY, Missouri (Reuters) - Firefighters on Wednesday resumed searching the rubble of a popular Kansas City, Missouri, restaurant destroyed by a natural gas blast and fire that injured up to 16 people and left a female employee missing.

Cadaver dogs explored the ruins of JJ's restaurant on the upscale Country Club Plaza shopping district overnight after the restaurant was engulfed by flames around 6 p.m. CST on Tuesday.

"At this time we are still searching for one victim and that is a female employee of the restaurant," Kansas City Fire Chief Paul Berardi said at press conference shortly before dawn.

Another employee reported missing late on Tuesday was found at an area hospital, he said.

Berardi said nine injured people were taken to hospitals and six or seven more went to hospitals on their own. Hospitals reported three people critically injured.

The restaurant and wine bar had several patrons inside at the time of the blast, Berardi said, along with employees. The explosion triggered a four-alarm fire that took 50 minutes to control.

Missouri Gas Energy said in a statement that first indications showed that a contractor doing work in the area had struck a natural gas line, but that the investigation was ongoing.

Bestselling self-help author Debbie Ford has died

SAN DIEGO (AP) ? The bestselling author of self-help book "The Dark Side of the Light Chasers" has died in San Diego. Debbie Ford was 57.

A family spokeswoman says Ford died Sunday in her home after a long battle with cancer.

"The Dark Side of the Light Chasers" spawned a self-help enterprise for Ford. The 1998 book aimed to help readers overcome their darker side.

Her sister, Arielle, said in a statement that the author wanted to help people break free from their emotional baggage and fear.

Ford wrote eight more books, led workshops and hosted TV and radio shows related to that topic. She often shared her battle with drug addiction, divorce and other personal struggles in her work.

She is also survived by her mother, son and brother.

The American Heart Association has reported that 25 million children in the US are overweight, which is one in three kids (one in seven are obese). The CDC has stated that the prevalence of childhood obesity has doubled in the last 20 years for children ages 5-11, and nearly tripled for teens ages 12-18. So, local pediatricians are stepping up to the plate and educating communities about healthy lifestyle. School menus are starting to get healthier, and the push for physical activity is growing. ?Childhood obesity is a growing problem in the United States here in Escambia County one of the ways they are trying to battle that problem is starting here in the schools by changing the lunches.?

Pediatrician Doctor Michelle Grier-Hall says obesity at a young age raises many health concerns not only physical but mental.

But CDC numbers nation wide show children ages 6 to 11 years who are obese increased from 7 % in 1980 to 18 % in 2010.

Adolescents ages 12 to 19 increased from 5 % to 18 %. Recently, schools have stepped in changing their lunch menus to healthier alternatives.

And removing friers from the cafeteria. ?There are five components in our meal whole grain bread, milk, fruit, vegetable, meat or meat alternate.?

Students say they appreciate the healthy change to their diets. And say the food isn?t so bad either.

?The quality of the food has gotten a lot better. We we?re pretty disappointed when it first started off and they were only going to do healthy food from now on and they got rid of the friers, but the fried chicken has got a lot better and the salads have gotten better.?

'A drop of ink on the luminous sky:' Wide Field Imager snaps cosmic gecko

Feb. 11, 2013 ? This part of the constellation of Sagittarius (The Archer) is one of the richest star fields in the whole sky -- the Large Sagittarius Star Cloud. The huge number of stars that light up this region dramatically emphasise the blackness of dark clouds like Barnard 86, which appears at the centre of this new picture from the Wide Field Imager, an instrument mounted on the MPG/ESO 2.2-metre telescope at ESO's La Silla Observatory in Chile.

This object, a small, isolated dark nebula known as a Bok globule [1], was described as "a drop of ink on the luminous sky" by its discoverer Edward Emerson Barnard [2], an American astronomer who discovered and photographed numerous comets, dark nebulae, one of Jupiter's moons, and made many other contributions. An exceptional visual observer and keen astrophotographer, Barnard was the first to use long-exposure photography to explore dark nebulae.

Through a small telescope Barnard 86 looks like a dearth of stars, or a window onto a patch of distant, clearer sky. However, this object is actually in the foreground of the star field -- a cold, dark, dense cloud made up of small dust grains that block starlight and make the region appear opaque. It is thought to have formed from the remnants of a molecular cloud that collapsed to form the nearby star cluster NGC 6520, seen just to the left of Barnard 86 in this image.

NGC 6520 is an open star cluster that contains many hot stars that glow bright blue-white, a telltale sign of their youth. Open clusters usually contain a few thousand stars that all formed at the same time, giving them all the same age. Such clusters usually only live comparatively short lives, on the order of several hundred million years, before drifting apart.

The incredible number of stars in this area of the sky muddles observations of this cluster, making it difficult to learn much about it. NGC 6520's age is thought to be around 150 million years, and both this star cluster and its dusty neighbour are thought to lie at a distance of around 6000 light-years from our Sun.

The stars that appear to be within Barnard 86 in the image above are in fact in front of it, lying between us and the dark cloud. Although it is not certain whether this is still happening within Barnard 86, many dark nebulae are known to have new stars forming in their centres -- as seen in the famous Horsehead Nebula, the striking object Lupus 3 (eso1303) and to a lesser extent in another of Barnard's discoveries, the Pipe Nebula. However, the light from the youngest stars is blocked by the surrounding dusty regions, and they can only be seen in infrared or longer-wavelength light.

[1] Bok globules were first observed in the 1940s by astronomer Bart Bok. They are very cold, dark clouds of gas and dust that often have new stars forming at their centres. These globules are rich in dust that scatters and absorbs background light, so they are almost opaque to visible light.

Security: Explosive breakthrough in research on molecular recognition

Feb. 12, 2013 ? Ever wonder how sometimes people still get through security with explosives on their person? Research done in the University of Alberta's Department of Chemical and Materials Engineering has revealed a new way to better detect these molecules associated with explosive mixtures.

A team of researchers including post-doctoral fellows Seonghwan Kim, Dongkyu Lee and Xuchen Liu, with research associate Charles Van Neste, visiting professor, Sangmin Jeon from the Pohang University of Science and Technology (South Korea), and Department of Chemical and Materials Engineering professor Thomas Thundat, has found a method of using receptor-free nanomechanical infrared spectroscopy to increase recognition of chemical molecules in explosive mixtures.

Detecting trace amounts of explosives with mixed molecules presents a formidable challenge for sensors with chemical coatings. The nanomechanical infrared spectroscopy used by the Univesity of Alberta research team provides higher selectivity in molecular detection by measuring the photothermal effect of the absorbed molecules.

Thundat, who holds the Canadian Excellence Research Chair in Oil Sands Molecular Engineering, says the spectroscopy looks at the physical nature of the molecule and "even if there are mixed molecules, we can detect specific molecules using this method."

Seonghwan (Sam) Kim explained that conventional sensors based on coatings generally cannot detect specific molecules in complex mixtures if the concentration of interfering molecules is five times greater than the target molecules. The detection sensitivity and selectivity are drastically increased using the high-power infrared laser because the photothermal signal comes from the absorption of infrared photons and nonradiative decay processes. Using this method, a few trillionths of a gram of explosive molecules can now be detected in a complex mixture even if there is a higher concentration of other interfering molecules.

The research team's findings are published in Scientific Reports by Nature Publishing Group on January 23, 2013.

The research team's current work looks at detecting biomolecules and hydrocarbons in the oil industry and nerve gas stimulants (DMMP), which can be found in household radiators, gasoline, and fabric softeners, for example. The team also hopes to develop a hand-held device for chemical detection that could be utilized in fields such as security, health care and environmental protection.

Lower autism risk with folic acid supplements in pregnancy

Feb. 12, 2013 ? Women who took folic acid supplements in early pregnancy almost halved the risk of having a child with autism. Beginning to take folic acid supplements later in pregnancy did not reduce the risk. This is shown in new findings from the ABC Study and Norwegian Mother and Child Cohort Study published in the Journal of The American Medical Association (JAMA).

Women who took folic acid supplements from four weeks before conception to eight weeks into pregnancy had a 40 per cent lower risk of giving birth to children with childhood autism (classic autism). Use of folic acid supplements midway through pregnancy (week 22) had no effect.

The findings only apply to a lower risk of childhood autism, the most severe form of autism. The results show no reduction in the risk of atypical or unspecific autism. The study also investigated the prevalence of Asperger syndrome, but the number of examined children was too low to give a reliable result.

Food and other supplements did not reduce risk

The researchers found no connection between childhood autism and intake of other supplements during pregnancy. They also found no correlation with maternal intake of folate through food.

"It appears that the reduced risk of childhood autism only reflects folic acid supplements, not food or other supplements, and that the crucial time interval is from four weeks before conception to eight weeks into pregnancy," says Dr P?l Sur?n, primary author of the paper and researcher at the Norwegian Institute of Public Health.

Clear results that pave the way for further research

The results show an association between the use of folic acid supplements in the mother during pregnancy and a reduced risk of childhood autism.

"The study does not prove that folic acid supplements can prevent childhood autism. However, the findings are so apparent that they constitute a good argument to further examine possible causal mechanisms. It should also be ascertained whether folic acid is associated with a reduced risk of other brain disorders in children," says Sur?n.

Emphasises the importance of folic acid supplements

The results support the Norwegian Directorate of Health's recommendations for folic acid supplements during pregnancy and emphasise the importance of starting early -- preferably before conception.

Method

The ABC Study included participants in the Norwegian Mother and Child Cohort Study (MoBa) who were born in 2002-2008, and included a total of 85,176 children. The mothers had given detailed information about their diet and the use of supplements in early pregnancy. Children with autism diagnoses in MoBa were identified through questionnaires, referrals from parents and health personnel and through links to the Norwegian Patient Register. When the analyses were done, 270 children with autism diagnoses were identified in the study population. Of these children, 114 children had autism, 56 had Asperger syndrome and 100 had atypical or unspecified autism.

The use of folic acid supplements in early pregnancy increased sharply from 2002 to 2008 among women who participated in the Norwegian Mother and Child Cohort Study. 43 per cent of mothers took folic acid supplements in 2002, while the percentage had risen to 85 per cent in 2008. However, many women began later than is desirable; only half of women who took folic acid supplements had begun before conception.

About the study

The ABC study is conducted by the Norwegian Institute of Public Health in collaboration with Columbia University in New York and the National Institute of Neurological Disorders and Stroke (NINDS) in Bethesda, USA. The study received funding from NINDS. In addition, funds for research analysis were provided by the Norwegian Research Council. MoBa is managed by the NIPH.

The Norwegian Directorate of Health recommends that women who are planning to become pregnant should take folic acid supplements from one month before conception and during the first three months of pregnancy. The recommendation is based on research showing that the use of folic acid supplements in early pregnancy protects the fetus from spina bifida and other neural tube defects.

In recent years, researchers have begun to investigate whether folic acid supplements may also have other beneficial effects on the development of the brain and spinal cord in the fetus. A study from the Norwegian Mother and Child Cohort Study showed that mothers who took folic acid supplements early in pregnancy halved the risk of having children with severe language delay at three years-old. A study of autism spectrum disorders from California found a lower risk of autism among children of expectant mothers who had taken multivitamin supplements containing folic acid.

"Argo," Warner Brothers capitalizing on awards at box office

LOS ANGELES (TheWrap.com) - "Argo," which seems to have been snatching up every award in sight lately, would be the front-runner in this one, too, if it existed: Best Use of a Sudden Slew of Honors.

Ben Affleck's Iran hostage thriller has been in release since October 12, but following the Oscar nominations on January 10 - and subsequent honors from the Producers Guild, Screen Actors Guild, Directors Guild and BAFTA - the box-office grosses have risen in four of the last five weeks. It's domestic total is $123.7 million, and nearly $14 million of that has come since the nominations - in weeks 14 through 18.

"Argo" established itself as a hit when it opened in 3,232 theaters, its largest distribution to date, to $19 million and two weeks later took the No. 1 weekend spot in its third week. It cemented that status in late November when it crossed the $100 million mark, but that was quite a while ago. And in the first week of 2013, "Argo" nearly dropped out of sight. It was down to 302 theaters, grossed less than $800,000 and fell out of the top 20 films.

But this past weekend, it grossed $2.5 million after Warner Bros. added 470 theaters to raise the location count to 1,405, and returned to the top ten for the first time since Thanksgiving. For some exhibitors, this is the second booking for "Argo," and that's unusual.

"It's an extraordinary situation," Warner Bros. executive vice president of domestic distribution Jeff Goldstein told TheWrap, "but then this has been an extraordinary ride for an extraordinary film."

Since its seven Oscar nominations, with Warner Bros. steadily upping its screen count, "Argo" has posted week-to-week percentage gains of 59, 80, 12 and, this past weekend, 23 percent.

Goldstein said that he would like to add more screens this week, but it could be tough because of the four openers crowding the market. On Valentine's Day, his studio is rolling out the drama "Beautiful Creatures," Relativity debuts its romance "Safe Haven," the Weinstein Company has its animated "Escape From Planet Earth" and Fox has "Good Day to Die Hard."

"Argo" comes out on DVD on February 19, so the post-nominations box-office run for the movie, which cost about $45 million to make, will likely slow. But there is, of course, one more chance for a bump with the Oscars on February 24, and Warner Bros. has shown it knows how to capitalize on hardware.

"Lincoln" is still leading the Best Picture nominee pack at the box office, with $174 million, followed by "Django Unchained," "Les Miserables," "Argo" and "Life of Pi." The international grosses of several of the films - "Pi" with $455 million, "Les Miz" with $215 million and "Django" with $187 million - top their domestic totals. "Argo" has made $75 million abroad.

There is no direct correlation between box office and Oscar success. Kathryn Bigelow's 2009 movie "The Hurt Locker" is Exhibit A. It beat out "Avatar" for Best Picture despite a domestic total of $17 million, compared to the $750 million total for James Cameron's 3D epic.

On the other hand, plenty of blockbusters - "Forrest Gump," "Titanic" and "Lord of the Rings" - have taken home the big prize. And 20 of the past 34 Best Picture winners have made $100 million or more at the box office, suggesting Academy voters can appreciate a film that resonates with a mass audience.

When you're on call, your employer decides exactly how long your leash is. That doesn't mean you can't stretch it, or see what you can get away with while meeting your company's expectations. A lot of this depends heavily on your office's policies, but here are a few things to start with:

  • How to Work an On Call Job and Keep Your Sanity"On call" should not be the same as "working." This is true for most companies, but if you get the feeling that in your job, being "on call" is the same as being "up all night waiting for something to happen so you may as well be working," you may need to have a chat with your manager. The point of "on call" work is, in most organizations, that there's someone available to jump in and work on an important issue after business hours?not that there's someone working around the clock that day. Similarly, make sure you're being paid appropriately for your on call and after-hours work. If you start racking up too many on call or overtime hours, odds are someone will sit up and take notice.   • You're allowed to disconnect sometimes, even when you're on call. This is something I learned from my days as an on call manager, and something I encouraged engineers I worked with to do as well. Even if you're on call, that doesn't mean you don't get to spend time with your family, eat a dinner without interruptions, or at least put the phone down for a little while?just let your coworkers know when you're doing it so they're aware. Communicate with your team and tell them you'll be offline for a half-hour or an hour while you sit down to dinner. If you can't get away with shutting your phone off for that time, at least mention it and ask only to be called if there's an emergency. Then follow-up at the end of your personal blackout period and make sure you didn't miss anything.
  • You're allowed to sleep. After a certain hour, email is no longer the best way to reach you, and if there's an emergency, someone should pick up the phone and call you directly. This follows on from the "on call is not always-on," principle, but it should be clear that there's a point during your on call cycle where email or IM stops being the best way to reach you. This is especially important if you're on call overnight: it's not unreasonable to say "I'm going to bed, if something comes up, call me."
  • You should have a clear procedure for when you're contacted, and a proper hand-off when your on call hours end. These kinds of policies sound tiresome, but they really serve two purposes: they protect you and your sanity, and they protect the efficiency of your team. Your customers, coworkers, and managers should have a reference for the kinds of things that they should wake you up for versus those that can wait until morning. Similarly, when your on call shift is over, you should be able to easily and seamlessly hand-off to someone else without worrying that you have to "finish up this one thing" or that someone on the team doesn't know you're no longer on call. If these things aren't clear on your team or at your company, talk to your boss and your colleagues. Your sanity is worth trying to improve them.
  • How to Work an On Call Job and Keep Your SanityFiercely defend your personal time. When you're not on call, there should be no real expectation that you'll be available unless it suits you or there's a real emergency. Most jobs, especially in technology or engineering, have some assumption that if your skills are required and it's an emergency that you should be reachable somehow within reason, but "within reason" doesn't mean "all the time." Defend your personal time by avoiding your work email when you're not on call, routing coworkers to voicemail when you need privacy, and turning off work-related alerts and notifications when you're off. Depending on your willpower, you may be able to do some of these things and not others, like leaving automatic alerts on but ignoring emails, so do what's right for you. Remember though, it's not a huge leap from "I'll just check my email before bed so I don't have so much in the morning" to "everyone at work knows I stay up until 2am working and happily exploit it." Most of these suggestions assume you work in an organization where "on call" means that you can be contacted by someone else at any time to look into an issue, not that you're expected to be up waiting for something to happen. At my last job, we had a network operations center that was staffed 24/7, and the crew would contact on-call technicians and managers when they were needed. Of course, they would only call if the issue met our criteria for severity, and once the on call staff were contacted, it set a chain of events into motion that would bring all the necessary people to the table to make sure the issue would be resolved as quickly as possible.

All in all, it was a good policy, but I'd worked at that job long enough to remember when "on call" was "okay it's your turn to stay up late and make sure things don't go down, respond to automated alerts pushed to your phone, and hope nothing happens when you try to sneak a few hours of sleep." If that sounds like your workplace, don't wait for someone else to fix it. Get together with your coworkers and your boss and shape it up yourself.

Use Technology to Reinforce Your Boundaries

The reason we advocate setting your boundaries so conservatively when it comes to your on and off time is because it's easier to slip back into bad habits if you try to take a metered approach to the issue. Of course, depending on your own self-discipline, you may be able to handle minor distractions or happily ignore emails and alerts knowing that if it's really important someone will call you. Some of you will have trouble with that, so turning everything off is the best option. Decide which is best for you, and embrace the tools, apps, and utilities that will help reinforce those boundaries.

Granted, it's not as simple as just "turning off your phone when you want some privacy." Here are a few ways to reinforce your boundaries when you're on call and when you're off:

  • How to Work an On Call Job and Keep Your SanityMake heavy use of your phone's do-not-disturb features. If you have an iPhone running iOS 6 or higher, you already have the tools for this. If you have an Android phone, you can set auto-responses or SMSes as a reply to unwanted phone calls, and apps like previously mentioned Silence can use your Google Calendar to determine when to turn your ringer on and when to turn it off, or you can set it yourself. Want more control? We like Sanity, an app that lets you control who rings through, even when your phone is on vibrate or silent mode.
  • Scale back (or shut off) your work-related notifications. You probably use your smartphone to check your work-related email, or have work-related alerts pushed to your personal device. You may even use your home computer to check your work email. Try to keep those things separate if you can (for example, I use one mail client on my Windows PC for work and another for everything else. When I shut off my work computer and end my workday, I don't want to even be tempted by my work inbox.) but if you can't, at least set your polling time for notifications, new emails, and other alerts back as far as you can. If you're on call and going home at night, every fifteen minutes to a half-hour is within reason, and won't annoy your family with a constantly buzzing phone all through dinner or an email client dinging every two or three minutes?all with messages you know you don't need to attend to. If you still need more control, Pushover is an amazing webapp that offers all the control you'll need.
  • Use mail filters and auto-responses as a first line of defense. If companies can do it, so can you. Canned responses to specific types of emails or specific people can be a lifesaver, even if that message says "I'm out of the office now, but if it's urgent contact me here," can go a long way towards changing a customer or coworker who emails on the weekend and expects a response into someone with firmly set expectations about when you'll get back in touch with them. I've known many engineers who put up out of offices when they go home at night, and others who use them whenever they know they'll be out of touch?even if they're not on call. "Going camping this weekend, so I won't be looking at email. I'll follow up with you on Monday, but if it's urgent you can call me here," for example. It may seem like a lot of information to give out, but if you're on call, "Thanks for sending this to me, I'll get back you in less than a half-hour" can make the difference between overwhelmed by emails and phone calls and everyone knowing their place in line.
  • How to Work an On Call Job and Keep Your SanityMake heavy use of vibrate settings, or buy a gadget that does. Being on call doesn't mean you have to wake up your family members when something at the office goes south, or heaven forbid deprive your spouse or children of sleep because you need to be able to wake if something happens. Sure, you could strap your phone to your arm and set it to vibrate, but there's no reason to go that far. There's a cottage industry of vibrating bracelets that go off silently when your phone rings. They're perfect for getting calls and getting up in the middle of the night without waking anyone else. If you're not eager to spend $50-100 for one, we're sure you can DIY your own system with a Bluetooth headset or even a pair of Bluetooth headphones to wake you (and only you) when you're needed. Similarly, if you just need to get up early to start an on call shift, or want to get up periodically to make sure everything is okay, use an alarm clock like the previously mentioned LARK silent alarm to wake you without stirring your partner. If you have a fitness tracking gadget that also tracks sleep, like the Fitbit or any of these devices we've tested, they can almost all wake you gently at the right time without waking up your whole house in the process. Address the Psychological Issue and Don't Deviate from Your Boundaries

Remember, you work to live, not live to work. Don't be afraid to get away, and don't be afraid to stick to your guns about the lines you draw between your personal and professional time. You're also in for a fight: you'll have coworkers who want their problems fixed right away, or who insist on your attention whenever they want it. You'll have managers who'll test those boundaries to see how far they can push you. Don't get frustrated, just stick to your guns and set clear, open expectations about what your behavior will be. Compromise when you have to.

When we talked about how to prioritize when everything is important, we explained that you can't do this alone. You'll need to build an understanding with your coworkers about how and when it's appropriate to get in touch with you, even when you're on call. The same applies to your family. You should also have an understanding with them that when you're on call, it means you'll be up at odd hours, you may have to go to the office on short notice, and sometimes you may have to miss dinner, or hole up in front of your computer working and shouldn't be disturbed.

When everyone is on board and aware of how you work, being on call won't be a struggle, it'll just be another habit or routine to fall into. Just make sure that when it's time to hang up your hat that you actually do it and mean it.

How I Gained It: While I?ve been an on-again off-again weight-lifter and runner since junior high, my eating habits were not the best, which usually kept my weight up regardless of how much I exercised. However, my weight shot up to 260 pounds in my second year of law school. My best friend from back home committed suicide, and this sent me into a depression. I hardly exercised and ate a lot of unhealthy foods (often deep fried, sweets and soda) to cope. Soon, my regular-sized clothes were no longer fitting properly and my studies were difficult to keep up with.

Breaking Point: After attending counseling to help deal with the depression, I began to feel a bit better emotionally. But when looking in the mirror one day, I realized that I allowed my health to slip away by gaining so much weight. I knew that I had to get back on the fitness wagon and stay with it no matter what.

How I Lost It: I joined a gym in order to get back into exercising and also started eating healthy again. I cut out a lot of sugar, virtually stopped drinking soda and hardly ordered out anymore. Instead of three large meals a day, I started having several smaller meals and snacks throughout the day. I began eating more fruit and vegetables and drank lots of water.

I also began to calculate calories in and calories out. Knowing that a pound was roughly 3,500 calories, I would count how many I was burning on the cardio machines, along with how many I would eat during the day. I started to make good progress with the treadmills and step climbers and weight lifting. Soon I was feeling better, more active and able to get back into some recreational sports. My clothes even started to get a bit too big for me. However, I eventually plateaued. I became bored with what I was doing. I needed something different. Then I finally took notice of the exercise classes at my gym.

I tried BODYPUMP, and quickly became a regular and then a hardcore participant, going to as many classes as I was able to all over the city. Then I started going to BODYCOMBAT and other classes, but it was BODYATTACK that was my favorite. I broke out of my plateau (as an average class burns more than 700 calories) and climbed, and continue to climb, a new mountain of healthy living and fitness. My instructors were motivating, encouraging and a lot of fun. Classes became a passion and a part of my life. Then one day I saw auditions for potential instructors. Given my acting background, zeal for classes, abundance of energy and desire to help others achieve their fitness goals and healthy lifestyles, I auditioned. After a weekend of training that was the most physically and mentally demanding experience in my life, nearly three months of practice and filming, and then submitting my video, I became a BODYATTACK instructor in fall 2011.

I thought that being a participant in a great class was one the best feelings ever, but being an instructor takes it to an incredible new level. I?m absolutely in love with what I do.

Clean-up begins after tornado mauls Mississippi

TUPELO, Mississippi (Reuters) - Authorities were cleaning up and assessing the damage on Monday after a number of tornadoes and severe weather tore through nine Mississippi counties, injuring dozens of people as homes and other buildings were torn apart.

Governor Phil Bryant said at least 63 people were injured and about 200 homes damaged or destroyed by the twisters, which cut a 75-mile path of destruction across the south-central portion of the state.

There were no reported deaths, but two people were critically injured when a tornado, believed to be at least a mile wide touched down in Hattiesburg, Mississippi, shortly before sunset on Sunday.

Preliminary data from the National Weather Service said the Hattiesburg tornado was an EF-3, packing destructive winds of up to 145 miles per hour.

Severely damaged buildings included the modern, multi-story Westminster Presbyterian Church in Hattiesburg, which flattened a pickup truck when debris flew off the building, and parts of the University of Southern Mississippi, Emergency Management spokesman Greg Flynn said.

He said most students at the university were off campus for the Mardi Gras holiday when the twister damaged several buildings there, including a performing arts center and an alumni house.

Bryant declared a state of emergency in counties hit by the severe weather and power outages continued across a widespread area as a steady rain fell on Monday.

"The bad thing is, it keeps raining," said Flynn. "It's supposed to rain all day today and then all day tomorrow.

"We've already had flash-flooding issues and the creeks and the streams are all overtopped. It's just going to make things a lot more difficult in the recovery process."

New genes for short-sightedness identified

An international team of scientists led by King's College London has discovered 24 new genes that cause refractive errors and myopia (short-sightedness).

Myopia is a major cause of blindness and visual impairment worldwide, and currently there is no cure. These findings, published today in the journal Nature Genetics, reveal genetic causes of the trait, which could lead to finding better treatments or ways of preventing the condition in the future.

Thirty per cent of Western populations and up to 80 per cent of Asian people suffer from myopia. During visual development in childhood and adolescence the eye grows in length, but in myopes it grows too long, and light entering the eye is then focused in front of the retina rather than on it. This results in a blurred image. This refractive error can be corrected with glasses, contact lenses or surgery. However, the eye remains longer, the retina is thinner, and this may lead to retinal detachment, glaucoma or macular degeneration, especially with higher degrees of myopia. Myopia is highly heritable, although up to now, little was known about the genetic background.

To find the genes responsible, researchers from Europe, Asia, Australia and the United States collaborated as the Consortium for Refraction and Myopia (CREAM). They analysed genetic and refractive error data of over 45,000 people from 32 different studies, and found 24 new genes for this trait, and confirmed two previously reported genes. Interestingly, the genes did not show significant differences between the European and Asian groups, despite the higher prevelance among Asian people. The new genes include those which function in brain and eye tissue signalling, the structure of the eye, and eye development. The genes lead to a high risk of myopia and carriers of the high-risk genes had a tenfold increased risk.

It was already known that environmental factors, such as reading, lack of outdoor exposure, and a higher level of education can increase the risk of myopia. The condition is more common in people living in urban areas. An unfavourable combination of genetic predisposition and environmental factors appears to be particularly risky for development of myopia. How these environmental factors affect the newly identified genes and cause myopia remains intriguing, and will be further investigated by the consortium.

Professor Chris Hammond from the Department of Twin Research and Genetic Epidemiology at King's College London, and lead author of the paper, said: 'We already knew that myopia ? or short-sightedness ? tends to run in families, but until now we knew little about the genetic causes. This study reveals for the first time a group of new genes that are associated with myopia and that carriers of some of these genes have a 10-fold increased risk of developing the condition.

'Currently myopia is corrected with glasses or contact lenses, but now we understand more about the genetic triggers for the condition we can begin to explore other ways to correct it or prevent progression. It is an extremely exciting step forward which could potentially lead to better treatments or prevention in the future for millions around the world.'

Currently, possibilities to reduce progression of myopia are very limited. While one drug, called atropine, may reduce progression, it dilates the pupil and causes problems with light sensitivity and difficulty with reading. New options are necessary. Chances are good that the insights gained from this study will provide openings for development of new strategies.

Music?s biggest artists will take the stage for the 55th Grammy Awards on Sunday night at 8 p.m. ET on CBS. Held in the Staples Center in Los Angeles, the event is one of music industry?s largest, featuring live performances from 21 different artists.

While the broadcast may only last a few hours, it?s the product of days and weeks of work from everyone from set designers to audio engineers -- not to mention the artists themselves.

We went behind the scenes for one of the show's last days of rehearsals to get a glimpse at what goes into the epic live broadcast.

Not rehearsed in order, artists were each given between an hour and an hour and a half of rehearsal time between Thursday and Saturday to try their act out on stage. During the rehearsals, they set audio levels and determine blocking -- that is, where they will be located on the stage as they perform. The first time the show will be run though in order will be from 10 a.m. - 2 p.m. PT on Sunday, just a few hours before the show goes live across the globe.

Tunisian pro-government supporters rally

(AP) ? Several thousand supporters of Tunisia's ruling moderate Islamist party rallied in the capital in a pro-government demonstration Saturday, a day after the funeral of an assassinated opposition politician. Protesters hurled insults at France, accusing the former colonial ruler of interfering in the North African country's politics.

The ruling Ennahda party had called for a show of support for the constitutional assembly, whose work on a new constitution suffered a severe setback after the killing of Chokri Belaid on Feb. 6 ? when leftist parties withdrew their participation. It said the demonstration would also protest "French interference" after comments earlier in the week by French Interior Minister Manuel Valls, who denounced Belaid's killing as an attack on "the values of Tunisia's Jasmine revolution."

Tunisians overthrew their long-ruling President Zine El Abidine Ben Ali in January 2011, kicking off the Arab Spring revolutions. In the two years since, Ennahda, a moderate Islamist party, won elections and has governed in a coalition with two secular parties.

Protesters denounced Valls' remarks, claiming they showed that France is interfering in Tunisia's internal affairs. Demonstrators gathered in front of the National Theater on Tunis' main boulevard, waving flags of the Ennahda party and shouting "Get out, France."

The thoroughfare was bustling, with cafes full and shops reopened after a general strike the previous day. Police in riot armor and plainclothes officers patrolled Saturday, but gone were the tear gas and running street battles.

Friday's funeral for Belaid drew hundreds of thousands of mourners chanting anti-government slogans into the capital's heavily policed streets.

Valls had said on Europe 1 radio on Thursday that Belaid was "one of the democrats and we must support these democrats so that the values of the Jasmine Revolution are not betrayed. There is an Islamic fascism rising everywhere, but this obscurantism must, of course, be condemned because it denies the democracy for which the Libyan, Tunisian and Egyptian people have fought."

Valls was clearly pointing the finger at Salafists, with their strict interpretation of Islam, who have come to the fore, and smeared Ennahda's moderate image. At least one black Salafi flag was spotted in the sea of white Ennahda flags at the demonstration, which took place several hundred meters from the well-guarded French Embassy.

Fathi Rhayem, a teacher, said the demonstration "shows the Tunisian people's desire to show that it is sovereign, it is independent and is no longer under French protection."

"We want to show that we want to live on equal terms with France, as friends with reciprocal interests but not like a dominant and a dominated. The policy of submission ... is finished now," Rhayem said, reflecting the mindset of the new Tunisia just over two years after it overthrew its autocratic ruler.

The events have added to the growing turmoil in Tunisia, where the transition from dictatorship to democracy has been shaken by religious divides, political wrangling and economic struggles. It's been a perilous stretch for a country many hoped would be a model for other post-revolutionary Arab states.

Belaid, who in his car outside his home, was shot dead while by an unknown assailant. Hours after his killing Wednesday, Prime Minister Hamadi Jebali said he would form a new, technocratic government to guide the country to elections ? but Ennahda, his own party, rejected that idea soon afterward.

Late Friday, Jebali renewed his proposal for a new government, which would be a key concession to the country's opposition. "I am convinced this is the best solution for the current situation in Tunisia," Jebali said, offering to resign if the elected assembly did not accept his proposed Cabinet.

Although Jebali said he was confident he could get Ennahda's support, his party's earlier rejection of the proposal exposed its own divisions between moderates and hardliners, and it remained unclear how the prime minister planned to pull enough support to his side.

But the coalition's failure to stem the country's economic crisis and stop the often-violent rise of hardline Salafi Muslims have drawn fierce criticism, especially from staunch secularists such as Belaid. He had also accused Ennahda of backing some of the political violence through its own goon squads.

Millions of roses get handed out on Valentine's Day. But growing roses has an environmental impact worse than many other crops.

Start with climate change: most roses in the U.S. and Europe are imported from warmer climes. All that flying and trucking adds thousands of metric tons of carbon dioxide to the atmosphere.

Then there's all the water needed to, well, water the flowers. And the runoff fouled by copious quantities of pesticides needed to make the roses look perfect.??

There's also the wildlife and workers poisoned by all that fumigation. Add to that habitat destruction where floral plantations displace native forest and wetlands.

Finally, there's the refrigeration needed to keep those blooms fresh. The electricity is often produced by burning fossil fuels, and the refrigerant gases also exacerbate climate change.

A more sustainable and, possibly, more romantic approach is to go with flowers certified by outfits like VeriFlora or, even better, whatever flowers are in season locally. Of course, that's not much help for those of us in wintry climes. Maybe try writing a poem. Let?s see: Roses are red, violets are blue?
